Requests you send through this website
The contact form on this website collects only the details you enter in it: your name, your contact details and, depending on the form, details about the visit you'd like to request, such as the service you need, a preferred date or time, how you'd like to be contacted, whether you're a new or existing patient, your insurance provider, how you heard about us, or a message. They are used only to respond to your request.
Requests are stored encrypted by our website provider, which handles them for us under a business associate agreement. They are never sold and never used for advertising.
To keep out automated spam, the form uses a security check from our website provider's security partner. It confirms that a person, not a bot, is sending the form. For that check it sees your browser and IP address; it never sees what you type into the form.
Please don't include details about your health in the form. The office will follow up with you directly.
Cookies and analytics
Aesthetic Eye Care uses analytics or advertising tools on some pages of this website. They may set cookies and see your browser, your IP address and the pages you visit. They never run on the page with the contact form, so they never see what you type into it. You can block cookies in your browser’s settings.
Emergencies

Text messages
The contact form has separate boxes to agree to text messages from Aesthetic Eye Care: one for appointment and account texts and one for marketing texts. They’re never checked for you, and you can send a request without them. If you check one, Aesthetic Eye Care keeps a record of what you agreed to, the exact wording you saw, and when.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
Reply STOP to any text to opt out, or HELP for help. Message frequency varies, and message and data rates may apply. Carriers are not liable for delayed or undelivered messages.
Text messages aren’t fully secure, so Aesthetic Eye Care keeps health details out of them. Agreeing to texts is separate from, and doesn’t replace, any HIPAA authorization or consent to treatment. The full text message terms are part of the terms of use.
